Maybe there should be a duplicates check for access entries

Description

When an access entry is added, Tvh checks only if its id is unambiguous. I don't know which is the relation between id and username, password and others credentials, but id is not checked during user login...
So, could be more entries with same credentials and, when user logs in, tvh automatically choose the one that has more access privileges.
