Unable to Add Muxes Manually - I'm stuck

Hello,
I'm a bit stuck. I can't add Muxes manually because the button seems broken.
None of the built-in DVB Network regions seem to apply to me (I'm in Milwaukee, Wisconsin USA). The documentation suggests adding Mux(e) manually using the 'Add Mux(es) Manually...' button. When I click this button nothing but a JavaScript error occurs.
How might I add muxes manually? Is there a config file somewhere I can edit?
I know I have a good signal because I was able to configure MythTV (and previously Windows Media Center on the same box). W_Scan shows a number of multiplexes found. It seems my roadblock is not being able to add Muxes manually.

what browser are you using on what platform (mac, windows, linux)? and which version of tvheadend?

Latest version, 2.12. I've tried Safari, Firefox, and Chrome on a Mac and Firefox and Chrome on Linux.

this thing also happens to me, when i click on delivery system it gives me an error in log and nothing shows in that drop-down box.
Any hints?

Same thing here,
I need to ad muxes to get the HD channels in CanalDigital package for Sat as they are not in the "default list".

You can modify the source code a little bit. Look in the file,
linuxtv_muxes.h, easiest thing to do would be modify an existing
city to reflect the frequencies in your area, Say change Boston or
Philly for your local frequencies, then recompile it.
Your customized freqs will then be available.
If ya need more help than that please let me know...
I was unable to add a manual multiplex also, while setting up
a North American ATSC Frontend, Samsung 's5h1409' is my frontend.
Sounds like a user using an ATSC frontend cannot add a new mux manually.

I had problems too and found a solution.... in my case clicking "Add mixes manually" did actually bring up a window requesting initial tuning data... however clocking "Save" failed to do anything... what I didn't realise is that the frequency field box was expecting the frequency in kHz and not Hz... so it simply wasn't accepting the values... as soon as I took off 3 extra zeros from then end, off it went scanning for multiplexes...
