Problem with TBS6280 card with Linux driver

I am having problems with my TBS6280 dual DVB-T tuner card running with Tvheadend (v3.4.27) on my media server and XBMC running on a range of different platforms as clients. From a reboot of the media server everything works fine but after an indeterminate period of time (1 to 3 days usually) I am unable to play or record TV because all I get are a few broken pixels or green squares. In XBMC the buffer is very slow to load and keeps re-starting (and never finishing) and on Tvheadend via the web portal on 9981 I get the following continuity errors:

Which goes on until I switch off the channel. The problem seems to be down to the tuner card or Tvheadend as it occurs on all my XBMC clients at the same time and anyway is clear from directly accessing Tvheadend. My media server setup is with Ubuntu server 12.04 on an ASUS M5A78L mobo (with AMD 760G / AMD SB710 chipset) and an AMD FX4-4300 processor. However on a previous setup with the same card on a Gigabyte 73PVM mobo (Nvidia chipset) and Intel Pentium processor I had the same issue so it seems it’s not my hardware. I’m using the Linux driver from TBS (v14210) but also had the same problem on both hardware setups using the previous driver (v13… something or the other). I haven’t tried the latest driver v140323 yet although there is nothing in the notes to indicate this issue has been addressed.
Re-starting Tvheadend does not solve things. The only thing I can do is reboot the media server and then everything is fine for a while until it happens again in a few days.
I’ve checked both TBS and Tvheadend forums and noticed a few references to problems with continuity errors and some discussion about there being an incompatibility between the TBS6280 running under Linux and Tvheadend (or anyway a problem with the Linux driver). However I can’t find anything suggesting there is a solution.
I’d appreciate any help in understanding where the problem lies or at least how I can diagnose it properly. Are there any settings in Tvheadend I can tweak which would help (I’ve attached a screenshot of my tuner card settings although I’ve tried a few variations with idle scanning on or off etc. but to no avail)? In effect the system is unusable as I never know when it’s going to be working or not so I’d really like to solve this issue.

No longer supporting 3.4, please try master if you're having troubel and yes eventually it will be released as stable!
