TVHeadEnd recording at wrong times and not ending recordings seemingly randomly.

TVHeadEnd is randomly recording at the wrong times and randomly not ending recordings. Where do I begin to fix this? I have reinstalled OpenELEC, reinstalled TVHeadEnd, checked the EPG settings (using OTA). What can I do? I set recordings from the HTS webpage and then download and delete the recordings from the HTS webpage. I've noticed using some Android apps have caused a similar problem in the past so I stopped using them.
OpenELEC 4.2.1 on Raspberry Pi.

Just a couple questions I have.
- Are the recordings off by a predictable amount of time?
- Is it happening on multiple stations/networks/channels?
- Does the RPi system time appear to be ok?
You could check the option to include the recording time in the name so we can see what time TVHeadend was starting at. If you do that already what time does it say and what time should it have been?

- No, it seems to be random:
- A scheduled TV show will not be recorded and indicated "Time missed" but nothing else is scheduled or recorded at that time.
- A scheduled TV show will be recorded but indicated "Time missed" because another episode of the same TV show was being recorded at that time - for example Wednesday broadcast of Rising Damp recorded on Monday. Of course not physically possible unless HTS is capable of time travel!
- A scheduled TV show will start to record at the correct time but will not stop recording such that everything is recorded after (until stopped by myself - not run out of disk space yet).
- A TV show not scheduled to record will be recorded.
- I haven't checked every channel. It is definitely happening with UK channels ITV 3 and CBS Action (Rising Damp and Hawaii Five-O and Star Trek)
- Time on RPi is correct. I check periodically and on reboot.
What I have done is limit access to OpenELEC/RPi to one browser, Chrome as the problem may have started since using Firefox and Chromium on other systems. I have seen similar behaviour when accessing OpenELEC with Android apps so I'm hopeful this will solve the problem. Its only been a day so far and no more Rising Damp until Monday. Many thanks for your help and suggestions.
I have tried to add time stamp and other details to the file name but it hasn't worked. I'll fiddle with those settings again, Google them - probably I've missed something obvious.

Not working. The OpenELEC on the RPi monitor is claiming to be recording QI at 12:40 on 01/17/2015 on ITV2. That is wrong and it is currently 07:40 on 01/19/2015. Via samba I see in fact Rising Damp scheduled for 06:00 this morning has recorded. It is a mystery beyond me. Is it possible to buy a setup that works out-of-the-box?
According to OpenELEC on the RPi monitor the next scheduled recording is Frasier on BBC 1 on 16/01/2015.

One thing I noticed by accident today. How does the time look on your computer with Chrome installed? The time filters in the website seem to be based on the computer you're viewing from. Also where are you getting guide information from? Does it have the right time zone in it? Is there an xml you can upload with your guide info?

Thanks for your suggestions. The time in Chrome/TVHeadend webpage is correct. I would need instructions to find and upload an "xml". I have flashed OpenELEC to the SD card again, first time TVHeadend service did not appear in settings > add-ons > services. I flashed again and it was there. I have not enabled TVHeadend client this time, just the service as I stream recordings to other devices anyway. The EPG is at its default, I don't touch it - it just works. I wonder if I have faulty hardware as twice yesterday the DVB device powered down. It must be 2+ years that I've been using OpenELEC and TVHeadend without any problems other than those I suspect were caused by some Android apps. All TV shows recorded last night and this morning correctly. Fingers crossed...
