Help trouble shoot failed recordings(time missed) that really aren't failed

TVHeadend is used in the USA for OTA recordings only. Recently had to delete Network/Muxes/Services because of FCC repack not working correctly. Ran the setup wizard to re-scan and manually recreated all my recording rules
I'll include a screen shots of failed and completed lines in the UI. Also the actual recording rule. I have many of these (about 30 per/day) but all are being recorded

Looks like you had 2 same recordings scheduled at the same time. Check you upcoming recordings to see if you have duplicates. When this happens, one recording works, but the other fails usually because it cannot create the already created file.
Do you have multiple autorec entries?

Thanks for responding. Looking at upcoming recordings is a good clue what's going on. Looking at Autorec rules I don't see any duplicates
But I do see odd things in upcoming recordings. Namely many don't have the autorec icon showing. And if I click on the "info" icon it shows and invalid status. Attached is an example

If you sort by start time, the duplicates will be more clear.
I also have no idea why the duplicates have no channel name? Has your channel mapping or epg grabber channels changed???
In 4.3 there have been lots of fixes for autorec, so it is probably a good idea to upgrade. I think soon 4.3 will become the new stable 4.4, but not sure exactly when...
