Automatic tuning / channel scanning?

Hello!
I'm running into a bit of trouble, so I decided to break my questions down a bit.
I have issues setting up DVB-C, in particular getting muxes/services/channels to show.
Since the software is installed correctly, I know my way around Tvheadend a bit (I have a successfully set-up DVB-T and it's running fine), the signal is strong (tested on a TV), the hardware is fine (stick is recognized and loaded just fine), I believe the issue to lie with the pre-defined muxes to be outdated—that's why I'd love to scan for channels.
Now, I remember having read that some time ago one version or another of Tvheadend "broke" channel scanning / automatic tuning. Is that (still) so? (For the record, I'm running version 4.0.5.)
If it's not broken, how do I (correctly) initiate such a scan?
If it is indeed so, what's the alternative? Not selecting any pre-defined muxes during the setup and then manually adding them?
(Where do I find that data? Probably need to ask my cable provider? Where does Tvheadend save them, where exactly are the predefined muxes stored/hard-coded?)
